Kinship Radio Volunteer Waiver

 

 

I,          (name)         , desire to volunteer for Kinship Radio and perform the activities related to being a volunteer for station projects. I hereby voluntarily accept this Volunteer Waiver under the following terms:

  1. I, the Volunteer, release and hold harmless Kinship Radio from any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from my volunteer work with Kinship Radio.
  2. I hereby assume the risk of injury or harm in any activities performed as a volunteer and I release Kinship Radio from all liability for injury, illness, death, or property damage resulting from the activities of my time with Kinship Radio.
  3. I understand that Kinship Radio does not assume any responsibility for or obligation to provide financial assistance or other assistance, including but not limited to medical, health or disability insurance, in the event of injury, illness, death or property damage. I hereby release Kinship Radio from any claim whatsoever which arises or may arise in the future due to any first aid treatment or other medical services that are conducted in connection with an emergency during my time with Kinship Radio.
  4. Permission for Public Release: I grant Kinship Radio all rights, title, and interest in any and all photographic images and video or audio recordings that are made by Kinship Radio during my work with Kinship Radio, including, but not limited to, any royalties, proceeds, or other benefits that are derived from such photographs or recordings.
  5. Confidentiality Release: All volunteers have a responsibility to Kinship Radio, their listeners and donors to maintain the confidentiality of their circumstances and personal matters. I agree to the following:
    1. No identifying information about Kinship Radio listeners or donors (names, addresses, physical disabilities, etc.) will be revealed to anyone outside Kinship Radio and only to those Kinship Radio personnel to whom the information is necessary for service to the listener or donor.
    2. Discussing personal circumstances concerning a listener or donor, even if names, addresses are not revealed, is also considered a breach of confidentiality. Discussion of or description of any listener or donor’s personal information or circumstances is considered detrimental to the listener and donor’s rights to privacy.
    3. The fact that a listener’s name or donor’s name has been made public through the media does not alter the fact that the individual still has confidentiality privileges within Kinship Radio and its programs and agencies. Confidentiality must at all times be maintained.
  6. I understand that serving as a volunteer for Kinship Radio requires my successful completion of a background check and that Kinship Radio Administration will be contacting me via email and will conduct the background check on behalf of Kinship Radio.
